MOTION TO INTERVENE
Comes now the State of Arkansas, Office of Child Support Enforcement and for its
Motion states:
1. The Intervenor is involved in the establishment or enforcement of a support order
herein pursuant to its responsibilities under Ark. Code Ann. § 9-14-210(d) under
which JENNIFER A. POOLE, physical custodian, is the recipient of services under
Title IV-D of the Social Security Act.
That this case may result in the adjudication of a duty of support entered or to be
entered of record herein.
That Intervenor, pursuant to Federal Regulation and state law, is required to be
involved in the modification of the support obligation herein to an appropriate
amount, and enforcement of the support Order. Intervenor is so situated that the
disposition of this action may, as particular matter, impair or impede its ability to
protect its interest herein unless it is allowed to intervene.

4. That attached hereto is a copy of the pleading on which Intervenor seeks to pursue
action.
WHEREFORE, Intervenor prays the Court to enter its Order allowing the State of

3. Pursuant to Ark. Code Ann. § 9-14-107 as amended by Act 337 of 2003
JOHNNY A. HICKS should be required to provide to the custodial parent, and
when applicable, the Office of Child Support Enforcement proof of income for the
previous calendar year. Furthermore, JOHNNY A. HICKS should be required to
provide proof of previous years' income to the custodial parent by certified mail, but
not more than once per year, when requested in writing and sent by certified mail.
JOHNNYA. HICKS should be ordered to secure and maintain health care
insurance coverage for the benefit of the child(ren) and supply the Office of Child
Support Enforcement with proof of such coverage, if not already provided.
An order of immediate income withholding for child support, child support arrears,
cost and fees should be granted pursuant to Ark. Code Ann. § 9-10-112 and §
9-14-218, including withholding from worker's compensation pursuant to Ark. Code
Ann. § 11-9-210, income withholding from unemployment benefits and income
withholding for medical support should be granted pursuant to Ark. Code Ann. §
9-14-501 et seq.
JOHNNY A. HICKS should be ordered to post a cash bond or security of sufficient
value to insure payment of child support.
The State of Arkansas, Office of Child Support Enforcement should be awarded a
reasonable attorney fee and JOHNNY A. HICKS should be ordered to pay all costs
attributed to this action.

MOTION TO MODIFY SUPPORT
Comes now the State of Arkansas, Office of Child Support Enforcement and for its
Motion states:
1. That JOHNNY A. HICKS was ordered to pay the sum of $ 83.00 WEEKLY for the
support of the child(ren) subject of this Court's order of OCTOBER 05, 2010.
That since the entry of the aforesaid Order, the circumstances of the parties have
changed; that JOHNNY A. HICKS's gross income as defined by Ark. Code Ann. §
9-14-107 has changed by more than twenty percent (20%) or has changed by
more than One Hundred Dollars ($100.00) per month, and such constitutes a
material change in circumstances sufficient for the Court to review and adjust
JOHNNY A. HICKS's child support obligation according to the family support chart,
minus appropriate deductions. Additionally, JOHNNY A. HICKS has accrued child
support arrearages in the amount of $2075.00 as of December 31, 2017 of which
the Office of Child Support Enforcement is requesting a Judgment for this
amount.

WHEREFORE, the Office of Child Support Enforcement prays that the Court enter an
