JUDGMENT AND ORDER GRANTING CHANGE OF NAME
NOW on this 22" day of January, 2019, comes on for hearing, Petition for Change of
Name. Petitioner, Tevin Michael Lee, by and through his Next Friend, Megan Nachole Borders,
appears in person and with counsel Aimee L. Morrison. Evidence heard. The Court being duly
advised of the facts and matters contained in said Petition for Change of Name makes the
following findings and orders:
1. That Petitioner’s present name is Tevin Michael Lee and his Social Security Number is
XXX-XX-9971. Petitioner’s full Social Security Number is contained in the confidential
information filed with this Court.
2. That the new name desired by him is Tevin Michael Borders.
3. That Petitioner desires to change his name because he wishes to have, and be known by,
the same last name as his mother, step-father, and siblings, with whom he currently
resides and will continue to reside, and he wishes to identify with his family by taking
their name Borders.
4. That the Petitioner currently resides at 3800 N. 28 Street, Ozark, Christian County,
Missouri, the county in which this Petition has been filed.
5. That the Petitioner was born in Jackson County, Missouri, is a minor under the age of
fourteen (14) years, and his full date of birth is contained in the confidential information
field with this Court.6. That Petitioner’s father’s name is Ronnie Earl Lee, and his last known address is 512
Meadowlark, Apt. 10, Clinton, Missouri 64735.
7. That Petitioner is not married.
8. That Petitioner has no children.
9. That Petitioner’s name has not previously been changed.
10. That there is no unsatisfied judgment for money pending against Petitioner.
11. That there is no action for money presently pending against Petitioner.
12. That the change of name will not be detrimental to any other person, and it is in the best
interest of the minor child.
IT IS THEREFORE ORDERED, ADJUDGED AND DECREED that Petitioner’s
name is hereby legally changed to Tevin Michael Borders, and the Clerk of this Court is directed
to publish notice of said name change in accordance with Missouri Supreme Court Rule 95.05.
The Court further directs the Missouri Department of Health, Bureau of Vital Statistics to amend
Petitioner’s birth certificate to so reflect the name change.
