COMES NOW the plaintiff, by and through its undersigned attorneys, and states as follows:

1. Norman Regional Health Systems provided goods and/or services for the defendant
through January 02, 2018.

2. The defendant has failed to pay for the goods and/or services.

3. The defendant is indebted to plaintiff, as assignee, in the principal amount of $7,885.75.

WHEREFORE, Plaintiff prays for judgment against the defendant as follows:

The principal amount of $7,885.75;
Prejudgment interest at the legal rate of 6% per annum as damages, which as of
February 15, 2022 totals $1,950.89 (15 O.S. § 266);
